Root Cause Analysis (RCA) Specialist

Duties & Responsibilities:

• Lead and facilitate detailed Root Cause Analysis for critical equipment failures and safety incidents.

• Use structured RCA methodologies (TapRooT, ICAM, 5‑Why, FTA) to identify technical, human, and systemic root causes.

• Develop recommendations and corrective actions with responsible departments.

• Maintain the RCA database, track implementation, and verify effectiveness of actions.

• Conduct post‑implementation reviews and monitor recurring failure indicators.

• Train internal teams on RCA techniques and investigative tools.

• Support Management of Change (MOC) reviews related to failure mitigation.

Competencies:

• Strong expertise in RCA tools (TapRooT, Apollo, etc.).

• Analytical thinking with ability to identify systemic causes.

• Excellent facilitation, interviewing, and communication skills.

• Technical understanding of equipment failure modes.

• Ability to evaluate evidence, timelines, logs, and technical documentation.

• Strong report writing and investigation documentation skills.

• Certified TapRooT® Investigator (Essential)

• Apollo Root Cause Methodology certification (optional)

• CRL

Minimum Requirements:

• Education:

Bachelor’s degree in Engineering (BE) or an equivalent qualification from a reputable university.

Preferably Mechanical, Electrical or Instrumentation

• Experience:

Min. 15   years relevant technical experience in engineering field in large Refinery or Petrochemical

Min. 5 years experience in RCA facilitation and defect elimination process is a must.
